Medicare Annual Wellness Visit

Effective January 1, 2011, Medicare beneficiaries are entitled to an Annual Wellness Visit. This visit is not subject to deductibles or co-insurance.

The Medicare Annual Wellness Visit is an assessment of your medical history, functional ability, preventative care, and risk factors. You can refer to this site for more information:

In order to complete this assessment, we would like to get some information from you prior to your visit.  Please click HERE for the questionnaire, please complete it and bring it to your appointment. This form will be reviewed with you as part of your Annual Wellness Visit.

The Annual Wellness Visit is not an “annual physical”, and is not intended to be a visit to assess a specific medical condition. If you would like these services, we may need to schedule a separate appointment for you. Please let us know if you have any questions.
