Insurance Plans


Alameda Family Physicians participates in many different insurance plans. We are participants in many preferred provider organizations (PPOs). We also participate in Managed Health Care Plans through our membership in the Affinity Medical Group IPA.

Insurance - We are part of Affinity Medical Group, a large network of physicians in the East Bay that contracts with the following HMO Plans:
  • Health Net, and Health Net Seniority Plus
  • Blue Shield of California
  • Blue Cross - California Care
  • Cigna
  • Pacificare
  • Aetna
We also accept patients with:
  • Tricare and Tricare for Life
  • Medicare (established patients only)
  • Most Preferred Provider Plans (PPOs)
